The Future of Nursing: Leading Change, Advancing Health

            The Future of Nursing: Leading Change, Advancing Health is a report that examines the nursing workforce. It states that to fulfill the needs of the current aging, diverse population and changing healthcare system, the role of nurses should change. It focuses on the intersection between patients’ healthcare needs and the preparedness of the nursing workforce. It intends to improve the quality of healthcare provided to Americans through the enhancement of contributions offered by nurses.

The Robert Wood Johnson Foundation (RWJF) committee presented its initiative to the Institute of Medicine in 2008. The mission of the RWJF committee was to develop a report with detailed recommendations on the future of nursing (Campbell et al., 2017). The recommendations would also include changes in the public and private policies at the Federal, State, and local levels. The RWJF committee focused on various issues before formulating its recommendations. The first issue was the need to reconceptualize nurses’ roles based on the present needs of society, technological changes, and the workforce shortage.

The second issue was exploring different innovative solutions in terms of professional knowledge advancement. Health care delivery innovative solutions were also examined. The third issue was recruiting and retaining competent nursing staff in all healthcare settings (Campbell et al., 2017). The fourth issue was the expansion of the nursing faculty, including increasing nursing institutions’ capacity. The restructuring of nursing education to create competent professionals to cater to current and future needs was also considered.

The RWJF committee was asked to explain the roles that nurses can assume to solve the increasing demand for quality healthcare services. IOM highlighted health sectors that needed urgent attention by the new healthcare policies. The highlighted sectors include primary care, chronic illnesses management, and wellness and prevention services (Campbell et al., 2017). The need for mental health care and palliative services was also recognized.

Four key messages structure the IOM Report recommendations. The first message is the need for nurses to practice to the full capacity of the training and education they receive. The second message is that an upgraded education system that fosters academic progression should be used to ensure that nurses achieve higher education and training levels (Hassmiller & Reinhard, 2019). The third message is that nurses should collaborate with physicians and all healthcare providers in redesigning healthcare service levels (Hassmiller & Reinhard, 2019). The fourth message is that for effective planning and making of policies to be accomplished, an improvement in data collection and information structure must occur.

The recommendations have transformed nursing practice in various ways. Nurses work in concert with other healthcare providers to provide quality patient care services. An example is inter-professional collaboration in the management of diabetes mellitus (Hassmiller & Reinhard, 2019). Several institutions that provide quality nursing education are present. These institutions offer master’s and doctorate programs enabling nurses to achieve high levels of education. The information infrastructure and data collection have improved with the popularization of Health Information Technology. Electronic Health Records are an example of Health Information Technology (Halamka & Tripathi, 2017). Currently, nurses offer services that reflect the full depth of their training. They are involved in health promotion services, disease prevention, and palliative services.

State-based coalitions aim at creating a healthier nation in all the States of America. The coalitions are managed by nurses and business partners. They conduct health campaigns at the local and state levels. They advocate for the need for communities to have representatives from health organizations (Veenema et al., 2017). Through these strategies, the coalitions transform health care and build create healthy communities. By transforming health care through nursing services, the State-based action coalitions advance the goals specified in the IOM report. Furthermore, it enables nurses to practice to the full extent of their training.

The Florida Action Coalition a formed in 2011. Its vision is to provide quality patient-centered nursing care. The Florida Action Coalition workforce center conducted Statewide surveys on the readiness of nurses for board service (Wood et al., 2016). The findings from this survey provided the background for training to develop Florida’s nurses’ readiness for board services. This initiative of a Statewide survey advances the nursing profession by sensitizing them on the availability of continuous professional development points. Through these surveys, nurses are most likely to enroll in different training programs that help them to develop their skills and professional knowledge.

The second initiative spearheaded by the Florida Action Coalition is conducting the Board Service initiative. The initiative provided training through online classes, webinars, and coaching (Wood et al., 2016). The Board Service Initiative also involves linking trained nurses with potential employers. It is estimated that 6 graduates from the program work on the policy-making board. This initiative advances the nursing profession by equipping nurses with relevant leadership skills. These skills will enable the nurses to discharge their services diligently and professionally.

Various barriers to the advancement of the nursing profession exist in Florida. The first barrier is the need for nurses to have a supervisory agreement with physicians licensed in Florida (Wood et al., 2016). The second barrier is not recognizing nurses as primary caregivers by Medicaid and Medicare (Wood et al., 2016). The other barrier in the State of Florida is the inability to involuntarily commit a suicidal patient under the Baker Act. These barriers prevent the nursing practitioner from practicing to the full extent of their training and education as stipulated by the RWJF recommendations.

The first way to remove these barriers is by advocating for Congress to expand the Medicare program. The program should cover registered nurse services. The Medicare program should be amended to allow registered nurses to conduct admission assessments. Furthermore, physicians’ reimbursement rates stipulated in the Affordable Care Act should be provided to advanced practice registered nurses.

The RWJF committee aimed to develop a report with detailed recommendations on the future of nursing. The recommendations entailed changes in the public and private policies at the Federal, State, and local levels. The IOM recommendations state that a nurse should practice to the full extent of his or her education. The need for collaboration with physicians and other healthcare providers is also highlighted.
