The Committee on the Robert Wood Johnson Foundation Initiative, on the Future of Nursing, in conjunction with the Institute of Medicine (IOM), developed a vision of transforming healthcare (IOM, 2011). By recognizing the demands and limitations of the current healthcare practice, they could develop enhanced care strategies. The aim of the committee was to introduce new health strategies that would enable the delivery of health services to diverse populations within the US. Therefore, the Robert Wood Johnson Foundation has managed to provide compassionate care to people of different economic statuses and ethnicity in the US.

Promoting quality healthcare among all populations is quite challenging, especially due to the scarcity of resources. The Johnson Foundation facilitates the provision of care and transformative care delivery through; reliable improvement of health outcomes, wellness promotion, and disease prevention. According to the envisioned future of nursing, the focus areas are primary prevention and primary care. Inter-professional care management and health coordination are the primary drivers in ensuring change in healthcare.

The cost of care is also an aspect of change. The envisioned plan aims to facilitate the provision of quality care at an affordable cost for all. Value reward and quality are the main areas addressed by the Johnson Foundation and not the volume of care as it was initially (Anderson et al., 2020). Besides improving accessibility to individuals, the vision is also centered on community and population health approaches. Therefore, the proposed healthcare system is responsive to the needs of individuals and their desires through the application of the patient-centered care approach.

Key Messages that Structure the IOM Report Recommendations

The Future of Nursing is a book by the Robert Wood Johnson Foundation Initiative that focuses on the nursing barriers that have contributed to the limited ability to contribute to the healthcare system (IOM, 2011). Some of the barriers noted include; the limited capacity of the nursing education system, regulatory restrictions on the nursing practice, lack of workforce data, and fragmentation of healthcare. The report was based on four key messages.

The first message was; Nurses should practice to the full extent of their education and training. Nurses are educated and highly trained to perform several nursing duties. Regulatory, historical, and policy barriers have prevented nurses from performing their full scope of activities (Polansky et al., 2017). Nursing regulations vary depending on various states, limiting their ability to perform. In many of them, nurses are denied the ability to clerk patients, prescribe, admit patients, order and evaluate screening tests, and assess patient conditions.

Nurses should achieve higher levels of education and training through an improved education system that promotes seamless academic progression. One of the best ways of transforming nursing is the use of nursing education. Through education improvement, nurses can work better with enhanced medical technology, analyze more complex information required in decision-making, and improve inter-professional management. To improve the strategy, the committee advocated for more degree nurses. Moreover, doctorate nurses should be increased by 50% (IOM, 2011). Nursing education should be a continuous process; all members should never stop increasing their knowledge through learning.

Nurses should be full partners, with physicians and other health professionals, in redesigning health care in the United States. Multi-disciplinary health management has several advantages, according to evidence-based practice in healthcare. Nursing transformation involves improvement in collaborative management. They are also required to involve themselves in leadership positions with heads of other departments. They should not become confined to bedside care only; instead, they should work hard to become involved with leaders from other professionals. Moreover, they should take part in decision-making and policy formulation. The two main strategies suggested in facilitating improvement of leadership roles were; funding from stakeholders and private investors to help nurses take part in leadership training and ensuring that all hospital heads ensure nurses are part of the team making decisions and policies.

The fourth message was on effective workforce planning and policy making require better data collection and an improved information infrastructure. Proper planning and organization require the collection of data. Data is important in the delegation of nursing roles and duties and resource allocation (Madden et al., 2017). The nurse managers must ensure that they perform regular data collection to determine the number of nurses in the institution and their roles.

Role of State-Based Action Coalitions

Access to quality healthcare can be greatly improved by using highly qualified registered nurses (RNs) and advanced nurse practitioners (ANPs). They are effective in the provision of primary, chronic, and transitional healthcare. The application of nurses serving in specialized areas of care and coordination has led to a decrease in the rates of hospitalizations and hospital readmissions. Healthcare workers (HCWs) empowerment is one of the ways of improving access to patient-centred care as the nurses are able to make better and more informed medical choices. Through the APNs’ education level, competencies, and skills, they are able to move beyond the existing barriers to healthcare delivery (IOM, 2011). Some barriers include; set health policies, outdated care regulations, and patient biases.

Another role nurses play in bridging the health gap and improving care access is the delivery of services at homes, workplaces, and schools. Inter-professional team management involves community health workers (CHWs) and occupational therapists. Through the use of CHWs and social workers, most of the underserved populations can access healthcare. As front-line workers, nurses require empowerment and support from governmental and private health stakeholders.

Per the IOM management guidelines, health value is the achievement of physical health and well-being in relation to the cost of care. There is less supporting evidence indicating that an increase in the scope of nursing practice may increase health delivery and access for society at large. Additionally, no evidence exists supporting the reduction of the cost of care in relation to the expansion of nursing care. Several studies are still being conducted to identify if scope widening can reduce the cost of care (Staff, 2020). The issue of health value is demonstrated with the reduction of antenatal hospitalizations and post-delivery complications for all patients who become delivered by a qualified midwife. Increasing the number of hours of management improves patient outcomes and reduces the chances of patient complications.

Initiatives on State’s Action Coalition

The national Campaign acts at both the national and state levels. It normally convenes several advisory and leadership agencies such as; the Diversity steering committee, Champion Nursing Coalition, Strategic Advisory Committee, and Champion Nursing Council. The aim of the agencies is to advance the goals of the IOM and campaign initiative. Meetings are convened by the Campaign to cover topics of the IOM report, such as education, leadership, and scope of education.

The Action Coalitions are considered the Campaign’s driving force as they enable local change. The first action coalitions were formed in November 2010 in New York, New Jersey, Michigan, Mississippi, and California (IOM, 2011). Through the State Implementation Program (SIP), the Campaign is able to provide funding for some of the coalitions. However, funding from the SIP can never be enough, and they encourage the coalitions to outsource. Other sources of funding may include; colleges and universities, the government, foundations, nursing organizations, and hospital systems.

State Barriers to Advancement

Various dashboard indicators were developed by the Campaign to help measure the progress. One of the barriers to advancement was limited data. It prevented the implementation of the campaign strategies. Another problem was the insignificance of the data presented. According to the IOM report, there should be a doubling of the number of APNs, which takes three years to study and even with their increment, there may be no significance in the nursing care workforce.
