Social and Community Context
Correctly identify your assigned social determinant. Briefly define the determinant and describe how it impacts healthcare.
“People’s relationships and interactions with family, friends, co-workers, and community members can have a major impact on their health and well-being” (Social and community context). These key characteristics are vital to understanding how people’s health is determined by the conditions in which they live, work and play. Conditions such as exposure to violence, unsafe neighbourhoods, access to transportation, playgrounds, the nearest grocery store and most importantly, access to health care. These challenges can all have a negative impact on health and safety throughout life. Additionally, factors such as those listed can affect an individual’s ability to take part in healthy behaviours. Healthy People 2030 focuses on helping people get the social support they need in the places where they live, work, learn, and play.
Select an objective related to your assigned social determinant from the Healthy People website linked above. Identify the status of the objective and the population to whom the objective applies.
The objective chosen for HP2030 is to eliminate very low food security among children. According to HP2030, 0.59% of households with children under 18 years had very low food security. Children in the United States with very low food security often don’t have enough to eat. This is directly linked to negative health outcomes, which leads to the possibility of children having trouble in school. Low food security is linked to low income. The amount of money people have has an effect on their health. “People with higher incomes tend to be healthier and live longer than people with low incomes. They have more access to grocery stores and healthy foods. People with low incomes can face challenges in getting healthy food.” (Staff et al., 2020). There are various measures in place that help combat low food security, which is one of the aims of HP2030.
Provide at least three appropriate questions related to the chosen objective that the NP could incorporate into the health history and interview to assess the client and family.
In the community in which you reside in, where is the nearest grocery store?
Does the grocery store in your community offer healthy foods at a reasonable price?
What resources are available to you and your family to help you in having access to healthy and nutritious food every day?
Identify at least two online, national, or local resources and explain how the resources could help meet the chosen objective for at-risk clients or families.
https://www.fns.usda.gov/snap/supplemental-nutrition-assistance-program
The following link provides individuals with low food security with resources such as SNAP which is a federal program that provides nutrition benefits and supplements to needy families so they can purchase healthy foods. SNAP, or Supplemental Nutrition Program, is a tool that the government has provided people with to help combat the issue of food insecurity. Individuals can use this link to check and see if they are eligible; once eligibility is confirmed, an application is filled out online. Once eligibility is confirmed, individuals are given a certain amount of benefits that they get access to every month, and with this, they are able to purchase healthy and nutritious food with assistance from the government.

This additional link provides another resource that the government provides people with. This specific program, known as WIC, is a specific nutrition program for Women, Infants, and Children. “Pregnant and postpartum women and children (under 5 years of age) participating in WIC receive food benefits for milk, cheese, eggs, cereals, peanut butter, dry beans/peas or canned beans/peas, and fruit or vegetable juices, fresh fruits and vegetables and whole grain choses to include breads, tortillas, brown rice, and oatmeal” (What does WIC provide? 2022).
