Telemedicine has dramatically increased access to patients who require diagnosis-focused care from a specialist. Offenders today have much more complicated medical issues that require the assistance of a specialist. Offenders would come to Health Services to be seen by the practitioner they would otherwise travel to see at UIHC via Telemedicine. Telemedicine would not eliminate all UIHC appointments, but it would save time and money and ensure the safety of staff, offenders, and the general public.

Stakeholders

Stakeholders in this project would include DOC and UIHC physicians, DOC nurses, nurse managers, and Fort Dodge Correctional Facility administration. The budget for this project would be created by the Deputy Warden of Administration. IT staff would be able to provide insight into the project’s technology. Offenders would be able to express their concerns and be reassured.

Outcomes/Efficiencies for Patients

A telemedicine program allows an offender to be seen for health issues in a more timely manner. The development of this program may result in fewer avoidable hospitalizations. Because the hospital and clinics provide extra security and a place to hold offenders while waiting for appointments, the time and money invested in that area would be reduced. When we transport offenders to the University of Iowa Hospital and Clinics, there is always a risk of injury. When an offender is ill, it can also be difficult to transport them. We reduce the risk of injury to staff, the public, or another offender by eliminating the need to transport all offenders who need appointments in specialty clinics. Offenders can see a specialist sooner and receive the care they require by using Telemedicine. Andrews (2018). The facility spends thousands of dollars a year on staffing trips to UIHC and could use those officers in-house, increasing overall safety. Implementing a telemedicine program in this facility would benefit not only the inmates but also the Department of Corrections.

Technology Necessary

HIPPA-compliant and secure technologies would be required. (n.d., How Do Telehealth and Telemedicine Technology Differ?) The facility’s IT department would be heavily involved in this project. The IT team, which would consist of two people, would set up the program’s technological aspects. Because this is a secure environment, the security director would decide where additional security cameras should be installed in the telemed area and develop security procedures as needed. Several computers with webcams in health services could be used to conduct offender consultations with University physicians. The purchase of medical equipment for use with Telemedicine would be required. During the visit, a nurse would always be present with the offender. Because the internet upgrade has already been completed, this would not be an additional cost for this project. When appointments are held over the web, the IT department must understand that priority bandwidth must be available.

Project Group

This project would be led by Nurse Informaticists. This person would create and implement work plans, design/develop the system, and analyze the project to ensure that the offenders and facility get the most out of it. (Spies, 2016) The NI would design the exam room and select the necessary equipment. Adding a nurse informaticist to the project team ensures that a knowledgeable member understands all aspects of the project. The project also includes the nurse director, a nurse, a physician, IT staff, and the security director.

Conclusion

The advancement of modern technology has expanded the possibilities for providing quality healthcare to offenders incarcerated in Iowa prisons. (2018) (Ng et al.) Telemedicine is the solution for both quality care and facility cost efficiency. Offenders can see a specialist sooner than if they had to travel three hours to UIHC. The facility will save a lot of money by not making unnecessary trips because offenders can visit the specialist online.
