Scenario

FR is a 30-year-old woman who has been complaining of generalized abdominal pain radiating to the back for the past week. The pain started out mild, but in the last two days, it has escalated to a 9/10. In addition, the patient reports pain during urination and a thin white discharge with an unpleasant odor during this time period.

Success and Difficulties

This week’s clinical experience was generally a success because I was able to introduce myself to the patient and become friendly with her, which allowed the patient to trust me. I assured her that whatever she said would be kept private and confidential. I had advanced in this field, so my ability to communicate with the patient and allow her to provide me with all of the necessary information to aid in diagnosis was critical. My success for the week was highlighted by effective communication with the patient. One challenge I still face is making a diagnosis based on the information provided by the patient. In these areas, I still need to seek advice and recommendations.

Patient evaluation

Based on the patient’s signs and symptoms, bacterial vaginosis (BV) unspecified is a possible diagnosis (B96). Lower abdominal pain, thin grey, white, or green vaginal discharge with an unpleasant odor, and a burning sensation during urination are all signs and symptoms of BV, according to Bhargava (2020). The patient exhibits the majority of these symptoms, including abdominal and back pain, urination pain, and thin white vaginal discharge with an unpleasant odor. As a result, the patient is most likely suffering from BV. The treatment plan would include both pharmacological and non-pharmacological approaches. For seven days, the patient would be given antibiotics such as Flagyl, 500 mg PO, and BID. According to Machado et al. (2015), non-pharmacological interventions for treating BV include yogurt and apple cider vinegar to balance vaginal pH, which leads to bacteria multiplication, probiotics to eliminate the high rates of bacteria in the vagina, and tea tree oil and water mixture to rinse the vagina and eliminate the unpleasant smell. In addition, the patient should be advised to improve her hygiene. Based on the patient’s signs and symptoms, differential diagnoses include vaginal candidiasis (B 37.3), painful urination (dysuria) (R30.0), and trichomoniasis (A59.9).

Learning Experience

Throughout the week, I gained valuable experience in effective communication in the delivery of care. This is significant in the future because it allows for the development of a positive attitude toward the patient as a care professional. As a result, I will strive to use effective communication in patient care, particularly during the first visit, because effective communication aids in proper diagnosis. Another important lesson is the value of collaborative care. I sought assistance in areas where I was unfamiliar, which aided in making the correct diagnosis and providing an effective treatment plan. Heise and Gilpin (2016) stress the significance of collaborative care in ensuring the quality and safety of care. As a result, in the future, I will always strive to foster collaboration among various healthcare professionals.
